What to Bring

Please bring your prescription glasses and current medications list for eye exams. If you're scheduled for a contact lens exam, don't forget your contact lens boxes.

  • Photo ID
  • Medical insurance card
  • Vision insurance information, if applicable
  • Your current prescription eyeglasses — please bring the pair you wear most often
  • Contact lens wearers — please wear your contacts to your appointment and bring your current contact lens boxes or packaging, if available
  • A current list of medications, including dosages
  • Any relevant eye-health or medical information
